Bosentan is a medication used to treat pulmonary arterial hypertension (PAH). It is a non-selective endothelin receptor antagonist that reverses the vasoconstrictive, proliferative, and profibrotic effects of endothelin-1, as well as influencing other growth factors and modulating ion channels in vascular smooth muscle cells. Oral bosentan is considered a first-line therapy for PAH, indicated for the treatment of World Health Organization (WHO) functional class III and IV idiopathic PAH or PAH associated with scleroderma.
