Molecular Formula: C29H30N6O6 (Note: The exact molecular weight may vary slightly in different sources, but is close to 558.59)
Appearance: Typically a pale yellow to off-white or white to grayish-white crystalline powder
Pharmacological Action
Olmesartan Medoxomil is a prodrug that, upon absorption in the gastrointestinal tract, is hydrolyzed to Olmesartan, which is a selective angiotensin II type 1 receptor (AT1) antagonist.
It blocks the binding of angiotensin II to the AT1 receptor on vascular smooth muscle, thereby inhibiting the vasoconstrictive effects of angiotensin II and achieving a reduction in blood pressure.
The hypotensive effect of Olmesartan Medoxomil is independent of the angiotensin II synthetic pathway and does not affect the degradation of bradykinin.
Clinical Applications
Olmesartan Medoxomil is primarily used for the treatment of hypertension, demonstrating efficacy across various types of hypertension.
Its notable features include a long half-life, allowing for effective blood pressure control over 24 hours with convenient dosing.
Compared to other angiotensin II receptor antagonists, Olmesartan Medoxomil offers advantages such as a smaller dose, faster onset, stronger and longer-lasting hypotensive effects, and a lower incidence of adverse reactions.
Additionally, Olmesartan Medoxomil exhibits beneficial effects on atherosclerosis, myocardial hypertrophy, heart failure, diabetes, nephropathy, among others.
Drug Interactions
Olmesartan Medoxomil is not metabolized by hepatic cytochrome P450 enzymes and does not affect P450 enzyme activity, thus avoiding drug interactions related to inhibition, induction, or metabolism of these enzymes.
No significant drug interactions were observed when Olmesartan Medoxomil was co-administered with digoxin or warfarin in healthy subjects.
Storage and Transportation
Olmesartan Medoxomil should be stored at -20°C to ensure stability and efficacy.
During transportation, measures such as ultra-low temperature ice packs should be used to maintain the low-temperature environment.
