Nateglinide is a non-sulfonylurea oral hypoglycemic agent belonging to the class of D-phenylalanine derivatives.

Pharmacological Action

Nateglinide stimulates insulin secretion by binding to sulfonylurea receptors on pancreatic β-cells, blocking ATP-sensitive potassium channels, leading to membrane depolarization, calcium channel opening, and ultimately promoting insulin release. Its hypoglycemic effect is dependent on blood glucose levels, resulting in a relatively low incidence of hypoglycemia. As a mealtime glucose regulator, Nateglinide effectively controls postprandial glucose levels with a rapid onset and short duration of action.

Indications

Nateglinide is indicated for the treatment of type 2 diabetes mellitus (T2DM) that is not adequately controlled by diet and exercise therapy or α-glucosidase inhibitors. It can be used alone in patients with T2DM who cannot achieve adequate glycemic control through diet and exercise or as an adjunct to metformin when metformin alone fails to provide adequate glycemic control. However, it should not be considered a substitute for metformin.
