Pharmacological Action

Sofosbuvir is a nucleotide analog used for the treatment of Hepatitis C.

  1. Mechanism of Action:

    • Sofosbuvir is a prodrug that, once metabolized in  the liver, is converted into the active antiviral agent, GS-461203 (also known as 2'-deoxy-2'-α-fluoro-β-C-methyluridine-5'-triphosphate).

    • This active metabolite acts as an inhibitor of the NS5B RNA-dependent RNA polymerase, which is essential for the viral  replication of Hepatitis C.

    • By incorporating into the HCV RNA chain during  replication, it causes chain termination, thus inhibiting the ability of the Hepatitis C virus to replicate its RNA.

  2. Clinical Use:

    • Sofosbuvir is highly effective when used in combination with other antiviral medications such as ribavirin, pegylated  interferon, and other direct-acting antivirals (DAAs) like ledipasvir.

    • It is used in various genotypes of Hepatitis C,  often achieving high cure rates.
